We ask for your cooperation and patience. Our #1 goal is to ensure the safety and
the health of our patients and staff.
If you have an upcoming appointment, please review the following screening
questions. If you answer “Yes” to any of the following questions, please contact the
office.
1. Have you been ill or had a fever in the last 14 days?
2. Have you been in contact with anyone who has been ill, had a fever, or had any other symptoms of coronavirus?
3. Have you had contact with anyone, either at home or at work, who has tested positive for coronavirus in the last 14 days?
If you have a scheduled appointment, please remain in your vehicle, and call the
office at 317-736-6361 upon your arrival. Per the governor’s mandate, we do ask
that you wear a mask in any common areas throughout the office. Upon your arrival,
we will take your temperature and have you sanitize your hands.
