Dear Patients and Friends,
Since our reopening on April 28th, we have been focused on keeping our patients, staff, and doctors- along with their families and close contacts- safe. This is a challenging task in the rapidly changing environment in which we currently live. Things look and feel a little different, but please take comfort knowing that all of these changes and precautions are intended for the highest level of safety for everyone. We ask for your cooperation and patience as we all navigate these uncharted waters together.
If you have an upcoming appointment, please review the following COVID screening questions. If your answer is “Yes” to any of these questions, please call and speak to a staff member for further instructions. We will ask these questions again prior to
your entry into the building.
1) Have you been ill or had a fever in the last 14 days?
2) Have you been in contact with anyone who has been ill, had a fever, or had any other symptoms of coronavirus?
3) Have you had contact with anyone, either at home or at work, who has tested positive for coronavirus in the last 14 days?
If you have a scheduled appointment, please remain in your vehicle, and call the office at 317-736-6361 upon your arrival. We will let you know when it is your turn to enter the building. Per the governor’s mandate, we do ask that you wear a mask in any common areas throughout the office until you are seated in the treatment area. Once you are in the building, we will take your temperature, have you sanitize your hands at the front desk, or wash them in the restroom.
